Bridging the Gap Event to Address Preventable Hospital Readmissions
Wintersville, Ohio – Sienna Nursing and Rehab will be hosting “Bridging the Gap” on Wednesday, May 8, 2019, from 5 to 7 p.m., at 250 Cadiz Road, Wintersville, Ohio. During this meet-and-greet forum, local healthcare professionals will share ideas for preventing unnecessary hospital readmissions and discuss how to better meet the educational needs of the community.
Tom Brennan of Breathe Technologies Inc. will be the featured speaker at the event. “Bridging Hospital to Home with WearABLE Open Ventilation” is the presentation and will be for 2 CEUs for RNs, RTs, SWs, and CCMs.
Now in its eighth year, Bridging the Gap brings together a wide range of healthcare professionals from throughout the Ohio Valley, including physicians, physician assistants, nurse practitioners, nurses, discharge planners, social workers, skilled care providers, home health agency providers, skilled care, hospice facilities, rehabilitation representatives, and DME representatives.
According to the Agency for Healthcare Research and Quality, one in five patients is readmitted to the hospital within 30 days of being discharged. That not only puts patients at risk for developing infections and other complications, but it also causes a financial strain for the healthcare system.
“Bridging the Gap has given participants an opportunity to discuss innovative strategies for reducing the number of preventable readmissions,” said Tracey L. Cutri, RN, CCM, director of case management at Acuity Specialty Hospital and organizer for Bridging the Gap. “Ultimately, this will help to improve patient care and improve the effectiveness and efficiency of the healthcare system. We look forward to sharing many more ideas at the next meet and greet forum.”
